2-Day On-Site Mock Survey

Assisted Living Consultants of Denver have developed comprehensive review tools designed to complete a 2-Day On-site Mock Survey. The Mock Survey utilizes CDPHE's  §§ 25-27-101, Colorado Revised Statutes, et seq., Assisted Living Residences - 6 CCR 1011-1, Chapter 7, Medication Administration  6 CCR 1011-1, Chapter 24, and General Licensure Standards - 6 CCR 1011-1, Chapter 2 regulations as guidance.  The Mock Survey includes the following audits:

    The community's 2-Day On-Site Mock Survey audit results, recommendations, and a final report will be provided upon completion.

    *2-Day On-Site Mock Survey includes travel to/from ALFs within the Denver metro area. Travel and lodging outside of the Denver metro area will be billed separately. 

  • Client Agreement

    Upon purchase, ALCD will provide the client, via email, with an agreement to be mutually signed. The signed agreement and full payment must be provided to ALCD before the client schedules the 2-Day On-Site Mock Survey with ALCD. The agreement ensures the 2-Day On-Site Mock Survey and the Mock Survey Audit Tools purchased and provided to clients is for the sole use of the client's owned, managed, and/or operated Assisted Living Facilities in Colorado. Clients agree not to, whether for a fee or without charge, reproduce, duplicate, copy, sell, transfer, trade, resell, re-provision, redistribute, or rent the Mock Survey Audit Tools except for appropriate and expected internal use.
